Go early and get uninterrupted views of down town the coast, spires etc. If you leave it later it is packed with cruise ship crowds blocking the platform listening to their guides. Late afternoon is a good time as well to visit.

We first visited this with our tour guide on our first morning in Tallinn. It was, unfortunately, full of cruiseship tourists and rather overcrowded. We came back later, on our own and it was worth it. You can get some really good photos from here....More

As others have said, the view is excellent, well worth the climb, if you can. There are roads, and it should be possible to take a cab from the lower town if you find the climb daunting.

I think the one in Kohtuotsa offer better view of both old and modern city view. this one was less crowded, though

Yes, you can go from Old town side - walk up the streets with no stairs.
